& the Dental Office of Dr. Robert Stephenson DDS,
are out to find the cutest kids in Western Oregon -
and reward one lucky child with a prize package valued at over $1000

Is your baby ready for the spotlight? Then enter them in “Who’s The Cutest of the All!” – but before you do keep the following in mind:
Child(ren) must be age 2 or younger as of May 6, 2013
Only one entry / photo per child please. Pictures can be either personal, candid shots, or professional photos (with copyright approval) but MUST HAVE BEEN TAKEN WITHIN THE PAST 60 DAYS.
ON-LINE ENTRY IS PREFERRED - but if needed you may register your child in person at the KDUK studios (1500 Valley River Drive Suite 350, Eugene, OR) during normal business hours (8am - 5pm) If submitting a hard photo please note that station can not guarantee that they will be returned to you - so choose carefully if you submit a picture that you can not easily replace!
Siblings are eligible to enter - but should submit seperate entry forms and pictures.
Only parents/ legal guardians are eligible to enter their child/ren in the contest.
Please note that due to the high number of entries, we do NOT respond to every entry but we WILL contact you should there be any issues / questions regarding your entry.
By entering your child you give KDUK the right to use your child's picture for promotional purposes, including posting on our website (www.kduk.com) and at the stations social media pages, including but not limited to, the KDUK facebook page (www.facebook.com/1047KDUKFM) for voting if your child is named to the Top 32.
CHILD MUST RESIDE IN KDUK'S LISTENING AREA...this includes all of Lane and portions of Douglas, Coos, Benton and Linn Counties as well. Questions of demographic elligibility are purely the discretion of KDUK judges.
OUR TOP 32 entires (as determined by a judges panel) will be announced on Monday, May 6, 2013 with KDUK’s Morning House Party. At that point the pictures will be posted for voting on line at KDUK dot com AND on the stations official facebook page (www.facebook.com/1047KDUKFM) and the field will be narrowed to our TOP 10 by public voting. All judges decisions are final. (Voting period: May 6 - May 10, 2013)
OUR TOP 10 entries (as determined by on-line voting) will be announced on Monday, May 13, 2013 with KDUK’s Morning House Party. At that point the tallies will be reset and the voting will resume to determine our Grand Prize Winner (Voting period May 13 - May 22, 2013)
Once open to voting ONLY ONE VOTE PER DAY PER PERSON- that is not per email, or per computer. Please note that the voting will be closely monitored each day and if any suspicion arises KDUK reserves the right to remove your child from the competition - please don't make this happen - let's keep things light-hearted! OUR “CUTEST OF THEM ALL!” (Grand Prize Winner) will be announced on Thursday, May 23, 2013 with KDUK’s Morning House Party. Parents of the Winning child will receive a prize packing including: (1) $500 Gift Card courtesy of Valley River Center, (1) Pamper Package courtesy of Blu Clark Salon (PPV $200), (1) Burch's Shoes gift certificate (PPV $200) and (1) DASH Professional Whitening from the Dental office of Dr. Robert Stevenson DDS (PPV $250) Additional prizes may be added at the stations discretion.
By completing this form I am submitting my child’s picture for consideration in the “CUTEST OF THEM ALL” contest being held by 104.7 KDUK. I release my child’s picture for use in the promotion of said contest, including it’s posting on the stations website and stations facebook page. I also agree that all decisions made in regards to the context by the KDUK staff, or it’s voting panel are final.
